That does seem the most likely situation. Or it was assumed that the broken open parcel had had that envelope in it when it was re-assembled. IT sounds like carelessness, but it'd be more likely to be a speed error than real cluelessness or illiteracy. Things can get quite hectic at a post office during primary processing hours.
